The deal spans six states — Texas, Montana, South Dakota, New Mexico, North Dakota, and Utah — marking one of the Salt Lake City-based company's most expansive single acquisitions to date.
Texas represents the bulk of the transaction, with 22 of the 34 facilities located in the Lone Star State. Montana contributes six facilities, South Dakota adds three, and New Mexico, North Dakota, and Utah each account for one facility. The geographic breadth of the deal underscores PACS Group's aggressive push to scale its skilled nursing footprint across the western United States.
